Child
Homemaker - Services consisting of general household activities (meal preparation and routine household care) provided by the trained homemaker, when the individual regularly responsible for these activities is temporarily absent or unable to manage the home and care for him or himself or others in the home.
Personal Care - Assistance with eating, bathing, dressing, personal hygiene, activities of daily living. Personal Care services may include the preparation of meals, exclusive of the cost of the meals. When specified in the plan of care, this service may also include such housekeeping chores as bed making, dusting and vacuuming, which are incidental to the care furnished, or which are essential to the health and welfare of the participant, rather than the participant's family.
Residential Habilitation Trainer - A trainer provides participant specific, individually designed and coordinated training. He or she assists participants in acquiring, retaining, and improving the self-help, socialization, adaptive, and safety skills necessary to reside successfully in home and community based settings.
Respite Care - Respite is provided to a participant who is unable, unassisted, to care for him or herself and the participant's primary care giver is absent or in need of relief from furnishing such services on a short-term basis pursuant to the individual plan of care.
Special Equipment: Initial & Repair - Services include devices, controls, or appliances, specified in the plan of care, which enable individuals to increase their abilities to perform activities of daily living, to perceive, control, or communicate with the environment in which they live. Items reimbursed with waiver funds shall exclude any medical equipment and supplies furnished under the Medicaid State Plan.

Adult Developmental Disability
Day Habilitation - Assistance with acquisition, retention, or improvement in self-help, socialization and adaptive skills, which take place in a non-residential setting, separate from the home or facility in which the individual resides.
In-Home Support - The provision of intermittent one-to-one habilitation services provided in the participant's home or the community to participants who reside with family, guardians, or independently. Individuals receive skills training to increase independence related to their own health care, self-care, safety, and access and use of community services.
Occupational Therapy - Services furnished by or under the scope of practice of an occupational therapist and necessary to keep a participant in his or her home and out of an institution.

* Residential Habilitation - the provision of habilitation services provided in the participant's home or in the community that provide assistance with acquisition, retention, or improvement in skills related to activities of daily living, such as personal grooming and cleanliness, bed making, and household chores, eating and the preparation of food, and the social and adaptive skills necessary to enable the individual to reside in a non-institutional setting. Residential habilitation services shall include access to residential habilitation services on a 24-hour basis.

Speech Therapy - Services prescribed by a physician and furnished by a speech pathologist, audiologist or under the scope of practice of a speech pathologist or audiologist including articulation, pragmatic language training and devices used by the participant. Services may also include assessment of the participant's use of visual cues, the participant's need for and use of amplification, and the participant's need for alternative speech output devices.
